Initial Comments:
It was determined during the yearly PRTF attestation project that this facility Bethesda Children's Home 6 Bed closed. A revised facility correspondence (dated and received July 10, 2023, at 1:30 p.m.) requesting closure; effective June 30, 2023, was accepted for this action. The purpose of this action was to discontinue participation under 42 CFR Part 483, Subpart G Regulations for Psychiatric Residential Tratment Facilities. The total number of beds closed under this provider agreement is eight (8) certified beds. The facility submitted the most current Certificate of Compliance in their possession; the span of March 11, 2023 until March 11, 2024. The legal name for this facility was Bethesda Children's Home (location indicator for our Division 6 bed) with the d/b/a of Bethesda Lutheran Services). The name located on the Certificate of Compliance is Bethesda Children's Home Lutheran Social Services Freeman-PRTF.
There were no deficiencies.
